The Statement of Intent – Integrated Health and Social Care Service in Powys

The statement of Intent – Integrated Health and Social Care Service in Powys is an important document produced by Powys County Council (PCC) and Powys teaching Health Board (PtHB) in collaboration with PAVO to provide seamless and integrated health and social care services for older people in Powys.

It forms one element of the One Powys Plan and sets out

· A baseline assessment in respect of health and social care service integration,
· Priorities and activities to strengthen service integration
· A framework for governance monitoring progress.

PAVO now attends the executive Health and Social Care Integrated Leadership Board (Carl Cooper) and the Integrated Care Pathways for Older People – ICPOP (Trish Buchan). Third Sector has already helped to shape decisions on the expenditure of Intermediate Care Fund through responses to the survey on integrated Care. As the work rolls out PAVO will be seeking more active involvement of our sector and will provide regular updates through the HSCN blogs.
