Community Support Coordinator
Grade: C32 (£23,076 pro rata)
Hours: 24.5 hours per week Base: Home based covering the Powys area
To provide a community support service to people living with HIV and/or Hepatitis C by carrying a caseload of clients and providing a direct support service on issues such as employment, treatments, welfare benefits, living well with HIV and/or Hep C.
You will also organise and facilitate peer support initiatives for people living with HIV and/or Hep C through groups, social events or one to one links and encourage participation in on-line forums, discussion groups and social networking sites to reduce isolation and increase participation
The post holder will also engage with local community organisations and identify barriers to participation for people living with HIV and/or Hep C, and design programmes to reduce these obstacles through for example designing and delivering awareness raising and training sessions.
