PAVO is seeking to recruit to the following posts:-
4-5 Community Health and Well-Being Coordinators
located in Brecon, Knighton and Presteigne, Llanidloes (tbc), Machynlleth and Ystradgynlais ,
working 28-30 hours per week over 4-5 days.
Fluent oral and written Welsh is required for the Machynlleth and Ystradgynlais posts.
The post holders will support adults, as part of integrated community based health and well-being support services, to access local community services and activities and services provided by the public, private and third sectors, by "brokering" their access to these services and working with the individual to find bespoke solutions to meet their personal needs. They will support people to access the right support, first time, in the right way and seek to empower them to build their own networks of support within their community.
Salary:- £19,773.60 - £21,186.00 per annum (£24,717 pro rata) funded until 31stMarch 2018
