Powys Association of Voluntary Organisations has the following vacancy
Community Connector (Builth Wells & Llanwrtyd Wells)
Based in Builth Wells
(required to work one day per week in Llandrindod Wells)
28 hours per week
Salary £20,370 per annum (£1,697 per month) £25,463 pro rata
(This role is funded until 31 March 2020)
The post holder will, as part of PAVO’s Community Health and Wellbeing Coordination team, work with statutory and other partners to provide front line support delivering integrated community based health and well-being support services that take a whole system approach to “what matters” to people, helping individuals, their families and carers to access the right support at the right time.
You will have a good knowledge of communities within Powys, experience of working alongside and supporting individuals and families from a variety of backgrounds and with varying needs, of working within multi-agency partnerships, delivering a person-centred service and supporting people to plan and meet their own needs for the future. 
 You will have excellent communication skills, be well organised, have good ICT skills and the ability to work using your own initiative, managing a demanding caseload.
Ability to communicate in Welsh is an advantage.
