IPC has been commissioned by the Department of Health and Social Care to identify good practice and innovation in providing care co-ordination or key working support for people (adults and children) with learning disabilities and long term conditions. The research will include a literature review of published information as well as exploring new and emerging models of co-ordinated care that aim to improve the health and wellbeing of people with learning disabilities.
