Powys Teaching Healthboard are launching an Advance Care Planning (ACP) document and project in Byw Nawr week, 13th-19th May 2019. The document, ‘My Life, My Wishes’ is currently being tested across Powys and we are keen to recruit ACP Champions from Powys Teaching Health Board, and voluntary organisations who would be happy to promote the documents, and also provide support to people completing it.
Full training for ACP champions is being offered on two dates, from 10.00 am -3.30pm
South Powys
Tuesday 12th March 2019, 10am-3.30pm Training Rooms 1 & 2, (opp. Library) Bronllys Hospital, Bronllys, Brecon, LD3 OLS. Refreshments will be available but you will need to bring your own lunch or order it from the canteen.
North Powys
Tuesday 19th March 2019. 10am – 3.30pm - MCRA, Plas Dolerw, Milford Road, Newtown, Powys, SY16 2EH Tea and coffee included but you will need to bring your own lunch, or there are shops/café’s nearby
