Third Sector and the Virtual Wards in Powys

The Virtual Ward is a GP led model which champions multi-agency working to care for people in their own home and prevent hospital admission. Older and frail patients, and those with complex care needs, can be referred onto the Virtual Ward and monitored more closely by a range of health and social care professionals including GP's, Nurses, Occupational therapists and Social Services. Voluntary Sector services are important in terms of supporting people within Virtual Wards and forming stronger links with GP surgeries.

The Virtual Ward model has been piloted in South Powys for nearly two years and was recently named overall winner of the NHS Wales Awards, as well as scooping the Improving Quality through Better Use of Resources Award. Due to this success, it is being rolled out now to GP surgeries in Mid and North Powys.

Currently, Powys Broker Service is representing the wider voluntary sector at each of these Virtual Ward meetings in the North as it develops. In Mid Powys, a few organisations share representation of the sector across the Virtual Wards and we hope that this format may be replicated in the North as developments progress.

Powys Broker Service also represents the voluntary sector within Powys People Direct which is a telephone hub for Social Services providing a single point of access for information, advice and referrals. The Broker Service works very closely with staff to ensure that, where needed, citizens have access to and knowledge of local community voluntary support services.

Family Anonymous Meeting

For Relatives & Friends Concerned about the use of Drugs, Alcohol or Related Behavioural Problems

DOES SOMEONE YOU CARE ABOUT USE DRUGS OR ALCOHOL?

What do you do if you discover, or even just suspect, that someone you love is taking drugs? Do you confront them? Do you seek advice? If so, who from? Police? School? A priest? Your doctor? The neighbours?

Drug or alcohol abuse is a very frightening and isolating problem and it helps to talk to people who know what it feels like to be in this situation. There is help available and there is hope.

Families Anonymous (FA) is a fellowship of self-help groups for families and friends of drug and alcohol abusers. Based on the famous ‘Twelve Steps’ of Alcoholics Anonymous, about 50 groups meet weekly all over the country.

A local group meets every 2nd and 4th Tuesday of the month

On behalf of the Powys Public Health Team, The Powys Annual Tobacco Control Conference 2015 will now be held on 17th November 2015, 10am – 3pm.

Aim:  Life Story Work is a tool to enhance the care provided to older people, particularly those with dementia. The benefits for individuals, families/friends and for staff providing care include improving understanding of the individual, promoting relationships and facilitating delivery of person-centred care. However despite increased emphasis on using life story work to support care delivery, there are often difficulties in implementation.

Older People's Commissioner for Wales Visit - 6 & 7 August

Iwan Williams the Local Government and Wellbeing Lead for the Older People's Commissioner for Wales will be travelling through Powys on his way to the Eisteddfod on 6th and 7th August and would like to visit some voluntary sector activities/clubs on his way to Meifod.

We will be collating a list of all of the activities happening in Powys on these dates for him to choose from. They are looking for opportunities to visit an older people's activity / club and meet the people taking part in these. So if your arts and craft group / luncheon club / befriending group etc are due to meet on these dates, please complete this form.

This is an excellent opportunity to highlight the fantastic work of the voluntary and community sector in Powys.

HSCN event 8 July

This particular event provided an opportunity to question Commissioners on procurement and commissioning strategies for Powys, to hear updates from the Welsh Government about new procurement regulations and how that would impact locally, to find out more how Primary Care services are interacting with the voluntary sector, and the key aims and objectives of Powys teaching Health Board, specifically in relation to working with the sector.

Commissioner seeks great ideas to keep road users safe

Police and Crime Commissioner Christopher Salmon has launched a scheme to save lives across Dyfed-Powys.  He wants to hear from organisations with great ideas on keeping road users safe across Carmarthenshire, Ceredigion, Pembrokeshire and Powys.

The best proposals will get a share of £300,000 to develop and run their schemes.

Mr Salmon said: “Too many people die or are seriously hurt on our roads; I want to change that.
“My Roads to Safety scheme will see innovative and exciting new road safety projects improve the lives of our communities and their road users.”

The fund will improve the safety of road users in Dyfed-Powys and reduce the number of road casualties across our communities. The Commissioner will make grants to public sector, police force, private sector, voluntary or community organisations and encourages organisations to work together.

A separate smaller budget of £35,000 is earmarked to look at specific research between academic institutions and partners, with a focus around emerging trends affecting road safety.
Mr Salmon aims to support projects that show a demonstration of innovation in building the research evidence base and applying it through knowledge exchange across all levels of road safety.

The Dyfed-Powys Independent (I) Advisory (A) Group (G) is recruiting!

An exciting opportunity has arisen for independent individuals to apply to become members of the Dyfed-Powys IAG. They are looking for membership of the IAG to be broadly representative of the diverse communities that they serve in terms of age, disability, gender, gender identity, race, religion or belief and sexual orientation.

The Dyfed-Powys Independent Advisory Group (IAG) provides the valuable role of critical friend to the Police Service and the Office of the Police and Crime Commissioner (OPCC). It is a forum where independent advisors can give independent advice about specific issues identified by the Police and the OPCC.

The role of the IAG is not one of scrutiny, but provides a safeguard against disadvantaging any section of our communities through a lack of understanding, ignorance or mistaken belief.

They are looking for individuals who are independent of the Police and willing to contribute time, energy and commitment to attend regular meetings; people who are committed to making a difference to the way that policing services are delivered.

The Welsh Government has had a great response to recent surveys which you may have completed (www.asdinfowales.co.uk/survey). Some parents/carers are keen to tell them more about their experiences and WG are keen to listen. WG have therefore arranged a number of focus groups in Wales for parents/carers of children and adults with ASD, one of which is to be hosted in Powys.

The groups will cover 2 areas:
1) Experiences of diagnostic assessment
2) Access to service provision for individuals with an ASD

They are seeking to find out about peoples experiences and ideas about what would help.

The groups are limited to 20 places, so they ask that only one parent/carer/partner represent each individual who has been diagnosed with an ASD.
