Dementia Action Plan for Powys

Powys t Health Board is leading on a joint action plan with Powys County Council  to strengthen support for people with dementia ( including in younger adults with  learning disability) and for their carers.
The action plan covers four main areas namely:
·                   Governance and Accountability
·                   Improving care in general wards in community
         hospitals
·                   Caring well in Care Homes
·                   Improving early assessment and care in communities
As the workstreams develop there will be opportunities for involvement of other professionals including in the Third Sector to contribute to task and finish groups. PAVO’s Health and Social Care Facilitator represents the sector on this group and will advertise opportunities as they arise.
One element of the action plan is to taking forward Dementia Supportive Communities. There is already a pilot project running in Brecon and district and the Dementia. A small task and finish group has been set up under the Dementia Action plan to take forward wider development of Dementia supportive Communities across the county and this group is planning a  workshop later this year to update and involve interested Third Sector organisations. If you would like to keep abreast with developments on Dementia please register your interest here
