The action plan covers four main areas namely:
One element of the action plan is to taking forward Dementia Supportive Communities. There is already a pilot project running in Brecon and district and the Dementia. A small task and finish group has been set up under the Dementia Action plan to take forward wider development of Dementia supportive Communities across the county and this group is planning a workshop later this year to update and involve interested Third Sector organisations. ·
Governance
and Accountability
·
Improving

hospitals
·
Caring

As the workstreams develop there will be
opportunities for involvement of other professionals including in the Third
Sector to contribute to task and finish groups. PAVO’s Health and Social Care
