Strengthening the Connections · Demonstration Events

Wednesday 9th April -South West Wales - Arts Hall, University of Wales Trinity St David, Lampeter, SA48 7ED - Delegates from Abertawe Bro Morgannwg, Hywel Dda and Powys health and social care communities
Strengthening the Connections’ exists to support the implementation of the Social Services and Well-Being (Wales) Bill and to place an emphasis on collaboration and integration between local government, NHS Wales and its partners. These free to attend ‘Demonstration Events’ will provide you with examples of best practice in relation to collaboration and integration between local government and NHS in Wales and elsewhere.  At the event you will: 
  •  hear from key practitioners about how to implement best practice in integration across Wales;
  • engage in open and creative conversations, to develop common understanding of the issues; and
  • explore the circumstances which both enable and prevent progress
Key speaker  Professor David Oliver Consultant Physician, Royal Berkshire NHS Foundation Trust; Visiting Fellow, the King’s Fund; President-elect, British Geriatrics Society; Former Dept. of Health National Clinical Director for Older Peoples Services (“Tsar”)
Workshops - Among others, the following workshops will be offered:*
Prof. Ian Philp, Medical Director, South Warwickshire NHS Foundation Trust – EASY-Care project on anticipatory care, with Age UK Warwickshire;
Linsey Reynolds, Age UK – Integrated Care Model;
Mark Shone, Cheshire Fire and Rescue Service – Data sharing to support integrated care; and Dr Claire Whitehead, GP, Sheffield – Integrated Care in Care Homes.
