Dementia in Action Strategy/Steering Group, developing
and co-producing strategy/action plan, Planning North Powys Dementia Event with Older People’s Commissioner
(office) and attending recent Alzheimer’s Society launch of a Wales Rural Dementia Network
Diabetes Delivery Plan, involved
in supporting this activity
Domiciliary Care Board, ensuring
the voice of third sector is fed into the development of the future Domiciliary
Care Strategy
Early Intervention/Prevention, feedback
on strategic approach (outline plan)
Intermediate Care Fund, proposals,
looking at gaps, service level agreements/reviews
Integrated Care Pathway for Older People, representing
Third Sector. Board is responsible for
implementing all aspects of Older People’s services and commissioning in Powys
Integrated Medium Term Plan (IMTP) with Powys teaching
Health Board, reviewing current IMTP, feedback on strategy,
liaising with Health Board about Third Sector involvement in shaping
strategy/future service delivery
Patient Experience Strategy Group, representing
Third Sector. Group is responsible for
ensuring the experience (voice) of patients is heard and acted upon.
Patient Experience Workshop (Engagement), attending
workshop and recruiting 8 patients to participate, feeding back on report.
Public Services Commissioning Training, currently
undertaking Public Services Commissioning accreditation in line with Joint
Commissioning strategies in Powys.
“Project” is on citizen engagement.
Meeting Sector Colleagues, supporting
Third Sector engagement with various strategies in Powys, reviewing
opportunities, reviewing service level agreements in relation to Intermediate
Care Funding, seeking views/feedback.
Meeting Senior Officer’s, HB/Council, seeking
to influence, promote and represent the Third Sector with key influencers in
Powys Councyt Council/Health Board.
Nursing Collaboration, second
meeting held with Nursing colleagues across Powys. CSSIW invited to join group. New Assistant Director of Nursing with Health
Board very enthusiastic about collaboration including joint recruiting,
potential rotation of staff, training, clinical supervision.
Older People’s Forum, attending
Forum listening to the views of Older People.
Powys Advocacy Network, Chairing
the Powys network, sharing information, picking up key themes//issues.
Public Health Wales, member of Strategic
Tobacco Control Group, recent feedback on strategy.
Social Services and Wellbeing Act Preparation, involved
in workshops, reviewing how prepared Sector is for Act, promoting engagement of
Sector.
Supporting People Management Board, looking
at recruiting Third Sector Representative to sit on Board, liaising re:
engagement on redesign of services, promoting involvement of mental health
organisations and individual representatives.
Third
Sector Broker Service:
Identifying
gaps in service. Brokers
recently identified the need for more volunteers in hospitals to provide
informal befriending for patients awaiting discharge. PAVO Health and Social Care team are
exploring this further.
Integrated
Assessments/Integrated Care Pathways. Heavily
involved in the planning for these integrated activities, attending joint
meetings with health and social care.
Key
statistics on people supported (enquiries/referrals) in
January 2016, 52. January 2015 figures, 35. Growth
of Broker enquiries/referrals is up 99.1% based on
last year and is continuing to grow.
Referrals also come from Third Sector organisations.
Powys
People Direct (PPD), central hub for all calls into Social
Services. The Brokers have a presence in
PPD every day and are passed referrals/enquiries directly from staff.
Prevention
(Third Sector Services) mapping. Compiled report of Third Sector services from
internal and external databases in preparation for the Social Services and
Wellbeing Act. Currently awaiting visual
maps to be produced by Council and will then circulate to Sector. Gaps are also being identified through this
activity.
Unscheduled
Care Board and Planned Care Board. Both
Brokers attend these Boards on behalf of PAVO each month, compile reports and
raise key themes/issues with H&SC/PAVO management.
Virtual
Wards, receiving direct referrals/enquiries from
statutory health services. The Brokers
are actively engaged/attending 10 virtual wards in North and South Powys every
week.

Communication
with Sector on a variety of specific subjects that arise
Community
Health Council (CHC)
leading on communication re: recruitment of members from Third Sector,
organising interviews, etc.
eBulletin:
Producing H&SC team’s eBulletin every two weeks, collating information,
responding to requests for circulating information.
Event
coordination/logistics, upcoming events being organised are
the Third Sector workshop on IMTP with the Health Board and North Powys
Dementia Supportive Communities.
Supporting
all H&SC activities, including arranging meetings with
key partners, participation of Third Sector organisations in statutory meetings
and providing website updates.
