MWHC Palliative & End of Life Care Scoping Exercise April to August 2016

Mid Wales Healthcare Collaborative Palliative and End of Life Care Scoping Exercise
April to August 2016

The Palliative and End of Life Care (PEoLC) Innovation Sub-Group of the Mid Wales Healthcare Collaborative (MWHC) is undertaking a short-term scoping exercise.

The aim of the project is to:

· Better understand current palliative and end of life care service provision for people in Mid Wales

· Identify gaps in service provision and barriers to access and how they might be addressed in order to improve the experience of patients, carers and their families.

The scope is NHS, Third Sector, Local Authority and Private Sector and the intention is that the project is both strategic and practical and will be of direct benefit to stakeholders in Mid-Wales.

They are keen that the scoping exercise benefits from a wide range of perspectives - patients, carers, volunteers, professionals. If you are interested in finding out more or contributing to this exercise please contact Catherine Smith, Senior Project Manager, phone: 0779 934264 , email: catherine.smith7@wales.nhs.uk
