Dementia Support Worker - Talgarth

The Alzheimers Society are looking for an exceptional individual to join their Dementia Support team. You will provide support and guidance to people with dementia and their carers across South Powys, helping to maintain their independence, improving their sense of well-being, and putting them more in control of their lives. Within this role it is essential that you build relationships with a wide and diverse range of local contacts including, but not limited to, GPs, mental health services, carers support services and other community based organisations. The service is unique to each service user as it is based upon their personal circumstances and support needs.

As a Dementia Support Worker you will organise and facilitate a regular Peer Support Group for carers aimed to inform, educate, and support those affected by dementia.
