Mid Powys Virtual Ward Workshops

PtHB would like to invite the voluntary sector to contribute to the plans for the roll out of the Virtual Ward to the Mid locality GP cluster.

If you provide health and social care support / services in Builth, Llandrindod Wells, Rhayader, Knighton or Presteigne, PtHB will be holding two workshops giving you the opportunity to discuss the roll out. The details are as follows

Group 1: Builth, Rhayader, Llandod. 2 June 2015 @ 13:30 Builth Wells Practice
Group 2: Presteigne & Knighton – 5 June 2015 @ 13:30 Knighton Hospital Seminar Room

Workshop 1. The workshop is expected to be between 3 and 4 hours in length. The idea is to
learn how to seamlessly work together in different and improving ways for the benefit of patients and community. From this workshop they intend to develop a draft operating policy, with everyone’s input.

The wider Multi-Disciplinary Teams are invited; Clinicians, Social and Third Sector to participate:

· GPs
· Practice Manager or nominated admin staff.
· District Nurses
· Social Worker
· Specialist Nurses
· Therapists
· Mental Health
· Third Sector

2. This wider MDT workshop will be followed by test and pilot sessions by the Virtual Ward core pillars: GP, Virtual Ward Clerk, DN Case Manager and Social Worker. This will be for the team to experiment and tailor the formal relationship to best fit.

3. Virtual Ward core team meeting with Local Implementation Team [LIT].

4. GP meeting with Locality GM and LIT to agree sign off and LES. GREEN LIGHT!
