Action on Hearing Loss

We help people confronting deafness, tinnitus and hearing loss to live the life they choose. We enable them to take control of their lives and remove the barriers in their way.

South and Mid Wales Outreach Service

South & Mid Wales Outreach is part of our Community Care & Support services.  It provides support to Deaf and Deafblind adults with additional needs such as Mental Health, Physical and Learning Disabilities who live independently in their own homes. We are based in Cardiff but we support people in Cardiff, Newport, Torfaen, Blaenau Gwent, Merthyr Tydfil, Rhondda Cynon Taff, Monmouthshire, Neath/Port Talbot, Caerphilly, Swansea, the Vale of Glamorgan and Powys.

We have a great and motivated team of support workers based in Llanelli and Cardiff who are committed to their job roles which they enjoy.

Every person using our service has their own history and their own aspirations for the future. That’s why we’re committed to support them draw up their own tailor-made programme of support based on their specific needs.  This involves working with them to develop a person centred plan and having members of the team as key workers to give continued and specialist support.

Our support workers enable people to be as independent as possible, supporting areas such as making healthy lifestyle choices, managing money, cooking, shopping, cleaning, using public transport and going to college or work. 

Based in and Powys, we are recruiting support workers to cover our Community and Outreach services.

We would love to meet someone who is caring, friendly and sociable who can make a difference to people’s lives. Being able to get out and about in the community is really important to people we support. We encourage people to live an independent life and to help make sure that they get the opportunities to do all the things we would like to. Mae Allgymorth De a Chanolbarth Cymru yn rhan o’n Gwasanaethau Gofal a Chymorth Cymunedol.  Mae’n darparu cymorth i oedolion Byddar a Dall a Byddar ac arnynt anghenion ychwanegol, megis Iechyd Meddwl, Anableddau Corfforol ac Anableddau Dysgu, sy’n byw’n annibynnol yn eu cartrefi’u hunain.  Rydym wedi’n lleoli yng Nghaerdydd, ond rydym yn cynorthwyo pobl yng Nghaerdydd, Casnewydd, Torfaen, Blaenau Gwent, Merthyr Tudful, Rhondda Cynon Taf, Sir Fynwy, Castell Nedd/Port Talbot, Caerffili, Abertawe, Bro Morgannwg a Phowys.

Mae gennym dîm gwych ac sydd wedi’i gymell yn dda o weithwyr cymorth sydd wedi’u lleoli yn Llanelli a Chaerdydd ac sydd wedi’u hymrwymo i rolau’u swyddi, swyddi y maent yn eu mwynhau.

Mae gan bob person sy’n defnyddio’n gwasanaeth ei hanes ei hun a’i ddyheadau’i hun ar gyfer y dyfodol.  Dyna pam rydym wedi’n hymrwymo i’w cynorthwyo i lunio’u rhaglen gymorth a luniwyd yn unswydd ac sydd wedi’i seilio ar eu hanghenion penodol.  Mae hyn yn golygu gweithio gyda nhw i ddatblygu cynllun sy’n canolbwyntio ar unigolion a chael aelodau’r tîm fel gweithwyr allweddol i roi cymorth parhaus ac arbenigol. 

Mae ein gweithwyr cymorth yn galluogi pobl i fod mor annibynnol â phosibl, gan gynorthwyo mewn meysydd megis gwneud dewisiadau iach o ran ffordd o fyw, rheoli arian, coginio, siopa, glanhau, defnyddio trafnidiaeth gyhoeddus, a mynd i’r coleg neu i’r gwaith.    

A ninnau wedi’n lleoli ym Mhowys a’r cyffiniau, rydym yn recriwtio gweithwyr cymorth i ymdrin â’n Gwasanaethau Cymunedol ac Allgymorth. 

Byddem wrth ein bodd o gyfarfod â rhywun sy’n ofalgar, yn gyfeillgar ac yn gymdeithasol a all wneud gwahaniaeth i fywydau pobl.  Mae bod yn gallu mynd yma ac acw yn y gymuned yn wirioneddol bwysig i bobl rydym yn eu cynorthwyo.  Anogwn bobl i fyw bywyd annibynnol ac i helpu i wneud yn sicr eu bod yn cael y cyfleoedd i wneud yr holl bethau y byddem ninnau’n hoffi’u gwneud.  Os oes gennych ddiddordeb mewn gweithio mewn tîm cefnogol, cofiwch wneud cais am y swydd hon.
